Do you want to participate?
If your child is aged 5-17 please complete the eligibility screener now to find out if they are a good fit for the study. You will need about 15 minutes to complete it. Here are some important tips:
- The online eligibility survey is meant for parents or legal guardians of children aged 5-17. If your child is under age 5, or 18 or older, please email our intake team for assistance.
- If you want to have more than one child evaluated, complete the online form again for each additional child you hope to enroll in the study.
- Once you complete the form, we will review the information you provided and get back to you with an eligibility update within two weeks.
- If your child is a good fit for the program, your name will be added to our waitlist.
